The Team
Hospice Medical Director and Physicians
Our Medical Director, together with our Attending Physicians, provide the primary link and involvement at every stage on the ongoing care of the Patient. They make sure the patient care wishes are honored above anything else.
Skilled Nurses and Aides
Registered Nurses and Licensed Vocational Nurses are trained specifically to deal with pain and symptom management, available 24 hours a day.
Certified Home Health Aides are available to assist with patient's personal needs, and help on their daily living activities including bathing, toileting, dressing, feeding, transfers, and ambulation.
Social Worker, Chaplain, and Volunteer
Social Workers are well-trained professionals available to help patient and families for counseling, bereavement services and community referrals.
Chaplains are available to provide our patient's, families, and caregivers with spiritual support if needed. They also play a key role in assisting with all areas of care, not limited to just spirituality.
Volunteers are available to provide companionship and assistance through visits, writing letters, legacy, building, reading, and shopping and other vital roles to meet community's needs.
